All Faith students wishing to participate on an athletic team must have an Athletic Participation Form and a Sport Physical Form on file with the school.
All students participating on an FLS athletic team or an after-school activity must fill out this participation form.
All students participating on an FLS athletic team must have a physician's exam once a year. This form should be filled out and signed by both a parent/legal guardian and a physician.
